All patients to be admitted to Winn Parish Medical Center will be registered in our CPSI information system and an Admission agreement form will be signed and insurance issues verified at that time. The Business Office/Registration Department of our medical center provides information to the patient regarding the Rights and Responsibilities of Patients and Advance Directives and will obtain appropriate signatures. If the patient has an Advance Directive, the Business Office/Registration Clerks in collaboration with our nursing staff assures that the advance directive is placed on the patient’s chart.
In the event that a patient is too ill to come to the Admitting Department prior to being admitted, a family member or surrogate decision maker can complete the process for the patient upon admission, or an admitting staff will complete the process of admission and bring the documentation to the bedside to review with the patient. If there is no family member with the patient on admission, a notice will be placed in the patient’s room asking family members to come to Admissions to complete any necessary paperwork. On the day of your registration, please bring the following items as they pertain to your specific situation:
- Insurance identification card(s)
- A photo identification card of the patient
- Social Security Card
- Living Will, if you have one
- Emergency notification of someone outside your home
- Minor patients must be accompanied by parent or legal guardian
We ask that you leave your valuables at home or send them home by family or friends. The hospital is not responsible for valuables kept in your room.
